Lianne – Social Worker #HeroesOfTransplantation

Hi, my name is Lianne and I’ve been the social worker with the liver transplant team at Freeman Hospital  for 13 years. I’m the only non-medical person on the team and it’s my job to support patients and their families with all aspects of practical, financial and emotional support and advice.

Hi, my name is Lianne and I’ve been the social worker with the liver transplant team at Freeman Hospital  for 13 years. I’m the only non-medical person on the team and it’s my job to support patients and their families with all aspects of practical, financial and emotional support and advice. I also co-host a weekly support group held via Zoom.

I love my job and the fact that no two days are the same. The resilience shown by patients who are waiting for a transplant astounds me every day. Of course, not everyone on the list gets transplanted, some people are just too sick and that can be hard, but I’m lucky that I work with such an amazing and supportive team.

The best part of my job is seeing people recover and go back to their normal life. I feel so lucky that I get to do this job, and hope that I make a difference to patients and their families. 

The role of a social worker in transplantation

Social workers are involved with transplant patients and living donors throughout the transplant and donation process including pre-evaluation, psychosocial informed consent, transplant and living donor selection, hospital discharge planning, short term and long term follow ups.
